Apply for funding in South Dakota.
Komen Great Plains will award community grants to organizations in South Dakota that will provide breast health and breast cancer projects that address funding priorities, which were selected based on data from the 2015 South Dakota Community Profile Report.
The funding priorities for the 2019-2020 Community Grants Cycle (in no particular order):
• Patient Navigation: Projects that provide evidence-based patient navigation for uninsured or underinsured individuals who reside in the Komen Great Plains service area, especially within target communities outlined in the Community Profile.
• Reducing Barriers to Care: Evidence-based projects that reduce barriers to quality breast cancer care experienced by uninsured and underinsured individuals residing in the Komen Great Plains service area, especially within target communities outlined in the Community Profile.
• Breast Cancer Education: Projects that provide evidence-based and culturally relevant breast cancer education in one-on-one and small group settings. Projects must be designed to result in documented age-appropriate action (e.g. getting a screening mammogram) These projects should target individuals who reside in the Komen Great Plains service area, especially within target communities outlined in the Community Profile.
To apply for a grant, please read the request for application (RFA) in full and apply through the Grants E-Management system (GeMS). An instruction manual for utilizing this system can be found below.

Komen Great Plains is now accepting applications for 2019-2020 Community Grants. All applications must be submitted online through the Komen Grants e-Management System (GeMS): https://affiliategrants.komen.org
